ABOUT K’AUTE PASIFIKA
K’aute Pasifika Trust is a well known Hamilton based community oriented organisation that is focused on supporting individuals and their families make meaningful changes to enhance their health and wellbeing. It is currently working through a transition period to deliver its services in new and different ways, through a new multi-disciplinary and multi-team approach. The new delivery model is strategically anchored and strongly outcome oriented and is focused on the organisation having a more integrated approach internally, and also working collaboratively with complementary external delivery partners.
THE OPPORTUNITY
As part of this refresh, we are creating two new functional manager positions within the Service Delivery team, each advertised separately:
- Manager Service Delivery (this role); and
- Manager Intake and Assessment.
Both roles will report to the newly established General Manager Service Delivery and will work closely together to ensure a seamless experience for aiga (families) from referral through to assessment, planning and delivery.
This role has been created to lead the planning and delivery function across K’aute, ensuring assessed needs are translated into coordinated, timely, culturally grounded and outcomes focused service delivery for aiga. You will provide functional leadership from assessment through to planning, delivery and outcomes, supporting effective service flow, responsive coordination and consistent delivery across multiple programmes and service areas, working closely with the Manager Intake and Assessment to ensure a smooth transition from referral, triage and assessment into coordinated planning and delivery.
ABOUT THE ROLE
This is a hands on operational leadership role. It will require strong clinical and/or practice-informed capability to support safe, effective and coordinated care, sound professional judgement in complex situations, and the ability to manage service demand, workflow, capacity and coordination across teams. You will translate assessment information into coordinated care plans and outcome focused delivery priorities, and use data and practice insight to monitor performance, identify trends and drive continuous improvement.
ABOUT YOU
To be considered for this role, you must have proven experience operating at a leadership level within a health or social services environment, with a track record of leading teams and coordinating service delivery at a system level, rather than within a single service or programme. A relevant tertiary qualification in health, social services or a related field is required, and professional registration in a relevant discipline (such as Social Work, Nursing, Counselling or Psychology) is strongly preferred.
You must be able to demonstrate a deep understanding of Pasifika people, and their cultures and values, along with strong relationship management skills to support integrated delivery across internal teams, external providers and community partners. It is essential that you bring strong communication and leadership capability and have shown that you can build and grow a high performing, collaborative and outcomes-focused delivery team.
WHY JOIN US
This is a unique opportunity for an experienced health or social services leader to take on a newly created role, and to play a key part in the strategic transformation and ongoing development of an organisation with significant aspirations for its future.
