About the RoleÌý
Chosen Family isÌýseekingÌýhighly skilled Nurse Practitioners (NPs) to provide advanced clinical assessment, care planning, and nursing leadership across disability and community services.Ìý
The NP will work autonomously and collaboratively to support participants with complex health,Ìýbehavioural, psychosocial, and disability-related needs.Ìý
Key ResponsibilitiesÌý
Advanced Clinical PracticeÌý
- Conduct advanced clinical assessments and diagnostic evaluationsÌýÌý
- Prescribe and manage medications within scope of practiceÌýÌý
- Develop advanced treatment plans and interventionsÌýÌý
- Support participants with:ÌýÌý
- Chronic health conditionsÌýÌý
- Complex disabilitiesÌýÌý
- Mental health presentationsÌýÌý
- BehaviouralÌýconcernsÌýÌý
- High-intensity support needsÌýÌý
- Provide advanced wound, continence, and chronic disease managementÌýÌý
- Coordinate referrals and specialist healthcare supportsÌýÌý
Clinical Leadership & GovernanceÌý
- Provide clinical leadership across nursing and disability support teamsÌýÌý
- Mentor Registered Nurses, ENs, and support workersÌýÌý
- Contribute to policy development and clinical governance frameworksÌýÌý
- Lead quality improvement andÌýevidence-based practice initiativesÌýÌý
- Participate in incident reviews and clinical risk management processesÌýÌý
Participant-CentredÌýCareÌý
- Promote participant dignity, autonomy, and informed decision-makingÌýÌý
- Deliver holistic, trauma-informed, and culturally safe careÌýÌý
- Support participant wellbeing, independence, and quality of lifeÌýÌý
- Facilitate collaborative care planning with participants and stakeholdersÌýÌý
Stakeholder EngagementÌý
- Liaise with GPs, hospitals, specialists, allied health professionals, and familiesÌýÌý
- Participate in multidisciplinary care coordination and case conferencingÌýÌý
- Build strong relationships with community and healthcare providersÌýÌý
Compliance & DocumentationÌý
- Maintain comprehensive clinical documentation and assessment recordsÌýÌý
- Ensure compliance with NDIS Practice Standards and clinical governance requirementsÌýÌý
- Participate in audits, reporting, and quality assurance activitiesÌýÌý
- Maintain privacy, confidentiality, and professional standardsÌýÌý
Essential CriteriaÌý
- Current AHPRA Registration as a Nurse PractitionerÌýÌý
- Extensive disability, community, or complex care experienceÌýÌý
- Current NDIS Worker Screening CheckÌýÌý
- Strong leadership and clinical governance skillsÌýÌý
- Excellent communication and stakeholder engagement abilitiesÌýÌý
Desirable CriteriaÌý
- Mental health NP endorsementÌýÌý
- Experience within disability health or psychosocial supportsÌýÌý
- Community outreach experienceÌýÌý
- Experience leading multidisciplinary teamsÌý
