About the RoleÌý
Chosen Family isÌýseekingÌýexperienced and compassionate Registered Nurses (RNs) to provide clinical nursingÌýsupportsÌýand disability services to participants across home, community, respite, and SIL settings.Ìý
The RN will play a key role in delivering and overseeing complexÌýsupportsÌýwhile ensuring participants receive safe, high-quality, and person-centredÌýcare tailored to their individual goals and needs.Ìý
Key ResponsibilitiesÌý
Clinical Nursing SupportsÌý
- Conduct comprehensive nursing assessments and participant reviewsÌýÌý
- Deliver complex clinical supports including:ÌýÌý
- PEG feedingÌýÌý
- Tracheostomy careÌýÌý
- Catheter managementÌýÌý
- Bowel careÌýÌý
- Wound managementÌýÌý
- Seizure managementÌýÌý
- Diabetes managementÌýÌý
- Enteral feedingÌýÌý
- Pressure injury managementÌýÌý
- Medication administration and managementÌýÌý
- Continence assessments and careÌýÌý
- Monitor participant health status and clinical outcomesÌýÌý
- IdentifyÌýchanges in participant condition and escalate appropriatelyÌýÌý
- Respond to medical incidents and emergency situations asÌýrequiredÌýÌý
- Ensure infection prevention and control standards are maintainedÌýÌý
Care Planning & Clinical OversightÌý
- Develop, implement, and review participant care plansÌýÌý
- Conduct clinical risk assessments and health reviewsÌýÌý
- Provide clinical oversight to support workers and Enrolled NursesÌýÌý
- Ensure safe delivery ofÌýhigh-intensityÌýsupportsÌýÌý
- Participate in incident management and safeguarding processesÌýÌý
- Support staff training and competency assessments whereÌýrequiredÌýÌý
- Promote evidence-based and best-practice nursing careÌýÌý
- Support positiveÌýbehaviourÌýsupport implementation whereÌýrequiredÌýÌý

- Promote participant dignity, independence, and choiceÌýÌý
- Support participants to achieve individual goals and improve quality of lifeÌýÌý

- Support participant emotional wellbeing and health literacyÌýÌý
Stakeholder EngagementÌý
- Liaise with families, guardians, Support Coordinators, and allied health professionalsÌýÌý
- Collaborate with multidisciplinary teams to ensure continuity of careÌýÌý
- Maintain professional and compassionate communicationÌýÌý
- Participate in case conferences and clinical reviewsÌýÌý
- Support positive relationships with participant support networksÌýÌý
Compliance & DocumentationÌý
- MaintainÌýaccurateÌýnursing and clinical documentationÌýÌý
- Complete medication charts, incident reports, and progress notesÌýÌý
- Ensure compliance with NDIS Practice Standards and clinical governance frameworksÌýÌý
- Participate in audits, reviews, and quality improvement initiativesÌýÌý
- Maintain participant privacy and confidentialityÌýÌý
- Ensure compliance withÌýorganisationalÌýpolicies and legislative requirementsÌýÌý
Essential CriteriaÌý
- Bachelor of NursingÌýÌý
- Current AHPRA Registration as a Registered NurseÌýÌý
- Current NDIS Worker Screening CheckÌýÌý
- Current First Aid & CPRÌýÌý
- Strong clinical assessment and documentation skillsÌýÌý
- Valid Driver’sÌýLicenceÌýÌý

- Disability nursing experienceÌýÌý
- Mental health orÌýbehaviouralÌýsupport experienceÌýÌý
- Experience within SIL or complex care environmentsÌýÌý
- High-intensity support competencyÌýÌý
