About the RoleÌý

As a Support Coordinator within our Capacity Team, you will help NDIS participants build capacity, connect withÌýsupports, developÌýindependenceÌýand navigate the complexities of the NDIS system confidently.Ìý

In this role, you will:Ìý

  • Deliver Support Coordination and Psychosocial Recovery CoachingÌý
  • Build strong, trusting and person-centredÌýrelationshipsÌý
  • Connect participants with NDIS, mainstream and community supportsÌý
  • Navigate barriers, respond toÌýcrisesÌýand support participants through changeÌý
  • Work collaboratively with Leads,ÌýpeersÌýand multidisciplinary professionalsÌý
  • Prepare reports, manageÌýdocumentationÌýand contribute to participant goalsÌý
