About the RoleÌý

We’re looking for an experienced and compassionate Care Partner to join us, working across home and community settings to support our growth in the Southeast region, Sydney.

This is a dynamic, field-based role — not a desk job. You’ll be out in the community, supporting clients in their homes and local settings, delivering high-quality, person-centred coordination and care.Ìý

Your portfolio will focus on clients funded under Supports at Home (Aged Care) ensuring support is personalised, respectful and aligned with funding requirements.Ìý

What You’ll Be DoingÌý

As a Care Partner, you’ll play a key role in:Ìý

• Conducting holistic assessments covering physical, psychosocial, cultural and environmental needsÌý
• Developing, implementing and reviewing individualised care plans built around each client’s goals, wellness, independence, safety and quality of lifeÌý
• Ensuring reassessments happen at regular intervals and key transition points — hospital discharge, change in condition or change in housingÌý
• Being the primary point of contact for clients, families and carers and advocating for their rights, dignity and preferencesÌý
• Overseeing services delivered by third-party providers for clients funded under Supports at HomeÌý
• Reporting accurately and on time to My Aged Care and other relevant agencies in line with program standardsÌý
• Growing your client portfolio by sharing the Leora story with clients, representatives and industry partnersÌý

About YouÌý

To succeed in this role, you’ll bring:Ìý

• A qualification in allied health, aged care, disability or a related community fieldÌýÌý
• Community engagement experience, including networking and stakeholder engagementÌýÌý
• Strong stakeholder management and relationship-building skillsÌýÌý
• Excellent verbal, written and presentation communicationÌýÌý
• Strong organisation and decision-making skills, especially in fast-paced environmentsÌýÌý
• A valid Australian Driver Licence (community travel is essential)ÌýÌý
• A genuine commitment to helping older Australians live well at homeÌý

You’ll also need a valid NDIS Worker Screening Check (or willingness to obtain one).Ìý
