About the RoleÌý

We’re looking for an experienced and compassionate Care Partner to join us, working across home and community settings to support our growth in the Southeast region, Sydney.Ìý

This is a dynamic, field-based role – not a desk job. You’ll be out in the community, supporting clients in their homes and local settings, delivering high-quality, person-centred coordination and care.Ìý

Your portfolio will focus on clients funded under the NDIS, including people living with disability and those recovering from injury under iCare NSW ensuring support is personalised, respectful and aligned with funding requirements.Ìý

What You’ll Be DoingÌý

As a Care Partner, you’ll play a key role in:Ìý

• Conducting holistic assessments covering physical, psychosocial, cultural and environmental needsÌý
• Developing, implementing and reviewing individualised care plans built around each client’s goals, wellness, safety and quality of lifeÌý
• Being the primary point of contact for clients, families and carers and advocating for their rights, dignity and preferencesÌý
• Running regular check-ins, progress reviews and site audits to maintain quality, safety and satisfactionÌý
• Monitoring client notes, incidents and service trends to spot risks early and escalate appropriatelyÌý
• Reporting accurately and on time to agencies (NDIA, iCare ) NSW in line with program standardsÌý
• Growing your client portfolio by sharing the Leora story with clients, representatives and industry partnersÌý

About YouÌý

To succeed in this role, you’ll bring:Ìý

• A qualification in allied health, aged care, disability or a related community fieldÌý
• Community engagement experience, including networking and stakeholder engagementÌý
• Strong stakeholder management and relationship-building skillsÌý
• Excellent verbal, written and presentation communicationÌý
• Strong organisation and decision-making skills, especially in fast-paced environmentsÌý
• A valid Australian Driver Licence (community travel is essential)Ìý
• A genuine commitment to improving the lives of people living with disability

You’ll also need a valid NDIS Worker Screening Check (or willingness to obtain one).Ìý
