Community Health Worker (50D)

2 x Male and 2 x Female Community Health Workers (0.5 FTE)

Part-Time Fixed Term | Remote Communities

Puntukurnu Aboriginal Medical Service (PAMS) is seeking Community Health Workers to support community members and families through health promotion, outreach, care navigation and early intervention activities. Working as part of the Upstream Health team, you will help identify barriers to wellbeing, connect people with services, and promote healthier communities.

About You

• Aboriginal and/or Torres Strait Islander person (identified position under Section 50(d))
• Respected member of the local community
• Strong communication and relationship-building skills
• Passion for supporting families and improving community wellbeing
• Current C Class Driver’s Licence or willingness to obtain one
• Willingness to undertake Community Health Worker training
