- We set you up for success, not burnout.ÌýWhile this is typically not your 9-to-5 role, you will start with a lower billable target and a warm pipeline ofÌýinitialÌýparticipant referrals, giving you the space to comfortably build your caseload over your first few months.Ìý

As a Support Coordinator, you will work holistically with participants across various age groups (children, young adults, and adults) to connect them with mainstream, community, and provider networks.Ìý
¸é±ð²õ±è´Ç²Ô²õ¾±²ú¾±±ô¾±³Ù¾±±ð²õ:Ìý
- Support participants to understand, implement, and maximize their NDIS plans.Ìý
- Budget planning and management to ensure participants get the most out of their plans for the life of the plan.Ìý
- Travel across Western Sydney asÌýrequiredÌýto conduct in person visits with participants and their support networks in their home.Ìý
- Foster capacity building, independence, and resilience within vulnerable participants.Ìý
- Coordinate,ÌýresearchÌýand follow upÌýmulti-agency service delivery, working closely with internal PBS practitioners and external providers.Ìý
- Conduct research and phone calls to providers to present participants with multiple optionsÌýforÌýproviders.Ìý
- Write comprehensiveÌýevidence-basedÌýprogress reports andÌýadvocate effectively for participants during NDIS plan reviews.Ìý
- Manage your caseload efficiently to meet the 25ÌýminimumÌýbillable hours per week milestone once fully ramped up.Ìý
- MaintainÌýaccurateÌýand up-to-date records of client interactions and progress.Ìý
¸é±ð±ç³Ü¾±°ù±ð³¾±ð²Ô³Ù²õ:ÌýÌý
- CurrentÌývalid Australian driver’s license and have access to a reliable, registered vehicle to perform community-based visits.Ìý
- Strong knowledge around disability, the NDIS ecosystem, and supporting vulnerable people.ÌýThis role requires empathy,Ìýcompassion,Ìýand understanding for people from allÌýwalks of life.Ìý
- ExcellentÌýwritten and verbal communication skills forÌýreport writingÌýand negotiationÌýto advocate effectively for your participants’ funding and needs, communication with providers andÌýeffectivelyÌýconduct research.Ìý
- Computer literacy for managingÌýspreadsheets, NDIS pages, internal CRM software, reportÌýwritingÌýand research.Ìý
- You love being part of a small team, are willing to share your own knowledge,ÌýresearchÌýand appreciate a workplace that values bothÌýhard workÌýand a positive, lighthearted atmosphere.Ìý
- Experience in a support or care role isÌýadvantageous, though entry-level candidates are encouraged to apply.Ìý
- Current NDIS Worker Screening Check, Employee/ paidÌýWorking with Children Check (WWCC), full work rights andÌýdriver’sÌýlicense.ÌýÌý
